REQUEST FOR PROPOSALS (RFP)
The Legacy Link, Inc., Area Agency on Aging (AAA) is announcing the Request for Proposals (RFP) for the State of Georgia, Fiscal Year 2028 (July 1, 2027 - June 30, 2028). We are accepting Statement(s) of Interest from organizations September 8 - September 25, 2026. Please submit a Statement of Interest including your organizations name, service provided, counties of interest, and contact information. This Statement of Interest should be email-ed directly to Lisa Howard, Contracts and Quality Director, at Lshoward@legacylink.org. Respondents will receive the schedule of events and the full RFP after the Statement of Interest period has closed. Proposals for one or more of the services listed below in the following counties served by The Legacy Link, Inc., will be considered:
Banks, Dawson, Forsyth, Franklin, Habersham, Hall, Hart, Lumpkin, Rabun, Stephens, Towns, Union, White.
Bid opportunities are available for the following services within the region: Evidence-based wellness services (bids may be to serve individual counties but must provide both programs): Bingocize, and Falls-Prevention - A Matter of Balance; Health Promotion/ Disease Prevention Services (to be provided throughout the region in the following categories: Nutrition Education, Lifestyle Management, Physical Activity, and Health Awareness/Disease Prevention); Senior Center Services in Towns and/or Union County with all services provided at a location that can support all of the following services: Congregate Meals and Health Promotion/Disease Prevention activities for older adults on-site participation, staging and facilitating delivery of Home Delivered Meals, and Transportation-Individual-Voucher Services to assist with travel within the county. In addition, complete intakes and assessments for all services, prepare and transmit reports, and store files per the US Department of Health and Human Services, Georgia Department of Human Services, as required, following all HIPAA compliance laws. All providers must be fully licensed to perform the service being provided and meet HIPAA compliance requirements.
